
This free funded programme extending to March 2026 is designed for students aged 10-18 living with, or affected by, a long-term health condition who may benefit from support with confidence building, self-management, emotional well-being, and planning for the future.
We’re looking for secondary schools to sign up from September 2025 onward.

What we offer
Group Work (Up to 8 weeks)
• Delivered in-school for typically an hour.
• Peer-based support in a safe, structured space.
• Focused on building resilience, self-advocacy, and shared experience.
1-to-1 mentoring (up to 12 weeks)
• Tailored, individual support
• Focused on student-led goals
• Practical support with transitions, confidence and health-related challenges.

All sessions are delivered in school during school hours.
