Target Group:
Young people aged 11–25 with long-term health conditions (LTHCs), especially those from racially minoritised and low-income backgrounds.
Programme Vision:
To transform life outcomes for young people with LTHCs by embedding youth-led, trauma-informed, and health-aware practices across education, healthcare and housing systems.
Programme Strands
There are six programme strands involved in My Life My Future 2.0
Return to Learn:
Trauma-informed re-entry into education after health-related absence or bereavement.
CYP Expert Patients Programme (CYP/EPP):
Peer-led health confidence and self-management programme.
MyHealth MyVoice Panels:
Youth-led governance and advocacy in school and hospitals Boards.
Youth-Led CPD & Undergraduate Degree Modules:
Communication training for professionals and healthcare undergraduates working with children and young people with Long-Term Health Conditions, co-designed and delivered by young people.
VR Learning Tools:
Immersive simulations of LTHC experiences for educators and social/healthcare professions.
Longitudinal Study:
7-year tracking of impact and youth-led storytelling.
Publish 7-year longitudinal study, together with evaluations and launch replication strategy.
DocuFilm:
Annual DocuFilms charting the progress of My Life My Future 2.0
Curtain-raiser of 30-minute docufilm of systems change process and programme
Years 1–2: Foundation & Pilot Phase:
- Establish partnerships with schools, NHS Trusts, universities & community orgs
- Launch Return to Learn and CYP/EPP pilots in 3–5 schools and 2–3 hospitals
- Recruit youth panels and peer researchers
- Begin longitudinal study with 30 young people
- Develop CPD modules and VR prototype
- Host inaugural Systems Change Symposium
- Annual DocuFilms
Years 3–7: Expansion & Legacy:
- Scale Return to Learn and CYP/EPP across boroughs and Trusts
- Embed youth voice in commissioning and governance, youth-led data collection and stories
- Pilot, then roll-out CPD modules and VR tools nationally
- Publish 7-year Longitudinal study, evaluations and launch replication strategy
- Final 30-minute documentary in Year 7
Evaluation & Learning:
- Peer-led impact evaluation with UCL and No More Box
- Economic modelling with LSE or UCL
- Youth-led data collection and policy briefings
- Annual docufilms and impact reports
